I might well be misremembering, and I definitely don't have my source materials nearby to double-check this - but in Mystara is there normal lycanthropy and then isn't there the super-lycanthropy that was messed about with by the Alphatian CDC which ended up being more common in the Known World? If I am remembering correctly then the answer is easy, base lycanthropy works like in 3.5E and the super-lycanthropy as augmented by the Alphatians is one that kills demi-humans*. Or that could just be a solution.

* maybe the Alphatians created a form of lycanthropy that would kill demi-humans as a way to get rid of those pesky dwarven wizards from Denwarf Hurgon :D

OK I'll just use the resource on the Vaults.

OK the CDC was trying to stop the Alphatian wizards who were messing around with lycanthropy and vampirism. Everything else I said could still stand however.

There have been several outbursts of lycanthropy, though they aren't explained or organized in the canon material.

As I recall. Mystara has natural Lycanthropys. These were shapeshifters from the same plane as the original Alphitians. It was a racial condition and not transferable. The Alphitians outbreak was super bad. One creature would infect another with just minor injuries and it jump species. Thus were-deer, were-mice, were-frogs, ect. The Alphitians pooled vast amounts of magic to help contain the nightmare they unleased. They linked the sickness to the moon and weaken it's transfer rate. Thus a were-boar could attack a deer and not spread it to the deer. Non-preditor versions died out. Natural Lycanthropys couldn't be effected by the sickness, but could become carriers. Thus on Mystara there are Naturals who can pass nothing, Naturals who are sick and can pass it, and the truely sick. Product material tells us some race can get certain varitys but not others. Mermen and tritions can get were-shark, but not other common types. Maybe demi-humans can get Lycanthropy, but only a certain type. Like Dwarfs being only capable of gaining were-bear or were-badger. This could easly lead to the belive that its always fatal. If the fatal rate was 98.9% this would also give the same concept.
