I've checked everywhere with no success. The DMG (1st edition, with the efreet cover) apparently had a printing prior to the one most of us are familiar with. It had the same cover but had a few differences in the layout and content. One of the most puzzling differences is that the list of contributing illustrators includes the name "Jeff Oleck"; his name is replaced in the revised edition by Erol Otus. The only illustration I found by Oleck is on p.40, and it was replaced in the revised edition. I have searched every list I can think of for Jeff Oleck but apparently he never worked on any other publication.
