Horror tracks was a 2e concept no longer used in 3e.

That said, it's not so hard but you'll have to do the work yourself. Simply pick a "normal" human and a monster from Libris Mortis. Then decide how many steps should be involved in the transformation of the former into the latter (10 steps works best usually). Finally, split the gradual changes between those steps and you're done.

As a side note, the Book of Shadows (which you can still download from the Kargatane website) has 20 examples of terror tracks.

Good luck and good work.

whats a horror track?

From the Book of Shadows Mortepierre referred to, "the ten-stage process introduced in Domains of Dread that can be used on heroes or nonplayer characters as a result of failed power checks." Basically, Horror Tracks could be developed for any humanoid-based monster in the Ravenloft setting, and with enough failed powers checks of a certain nature, the characters will gradually turn into it.

The idea of a horror track seems pretty cool, although it's rather limited. It could be used for one campaign maybe, and it should probably only be done for one character, or it'll lose its unique-ness.
