Is there a list of which gods reside in/on each plane?

See the sourcebook On Hallowed Ground which had pretty comprehensive lists of what gods realmed where in the 2e Great Wheel. Truly an awesome book if you can get a hold of it. It is pretty rare, and unfortunately has never been made available as a pdf for legal purchase. But if you can find it at a used bookstore or at an auction site, highly recommended that you pick it up.

Here is a link to an online list of planes, layers and divine realms, not sure how comprehensive it is, but I like to look at it from time to time:

Gods List, alas, doesn't list realms. Be nice if it did, of course.

The Gods list is quite comprehensive, I think. The only thing missing are the domains, but if you own Deities & Demigods and the FR Campaign Setting, that wouldn't be a problem. The Portfolio is usually quite easily translated into domains.

I've been asking Alzrius if he wanted to list realm locations by planes for the gods, but in what he's seen far too many of them just don't *have* that information listed in canon anywhere. If you are somewhat familar with the Planes though a look at the god's alignment, portfolio and/or pantheon should let you get approximately close to where the realm would be.

This information is all located in On Hallowed Ground. A rare book to be sure, but if you can get your hands on one, it lists pretty much every realm for every god (for 2e). It even has several comprehensive appendixes in the back with exhaustive lists.
