Actually, there are several different versions, not entirely compatible.
First, we know that the Great One belongs (or did in his mortal life, if you prefer) to an ancestor race of the Dragons, mid way between those quasi-dragons found in the Hollow World and the standard chromatic dragons.
So there is enough evidence that the dragons evolved in Mystara, possibly after the creation of the HW.

There's also the Eldar stuff, which IIRC comes from the dragonlord trilogy, which deals with the dragon origin, and says they are related to the elves -- except this version doesn't fit with the known origin of the elves...

In Frank Mentzer's original setup, dragons were an entirely different order of life and sapience entirely than humanoids, and achieved Immortal status and power through a completely different method. I see dragons and their like as a way to coalesce Immortal essence (and eventually, Old One status) directly from the primal nature of the Prime, as opposed to (demi-)human questing for Immortality which tries to put human will and desires behind the process.

In fact, if one accepts Sharon Dornfeldt's old idea about the sixth dimension 'branching off' from the 3rd, then dragons might exist in dimensions 1,2,3 and 6 - as opposed to the 1-4 of normal mortals. In this case, they'd be naturally capable of powers and insights beyond even the Immortals' normal ken... and even less able to comprehend the Nightmare dimension than we would be.

If that is the "true" origin for them, then it's possible to believe that they're actually related to Carnifex and not much older than the carnifex-era. And this would make Ka older than the Great One.

I guess this explains why linking the elves and the yuan-ti

Or any dragon-related races, like drakes, dragonnes, pseudo-dragons, wyverns and, why not, dinosaurs (a wyvern is not smarter than a dino)

We aren't sure. They eat anybody who tries to ask.

(Srsly, I suspect they're more a spiritual/conceptual cousin to the dragons than physically alike. The entire race managed to ascend into Immortal-level life orders aeons ago, using the primal nature of the Astral... and in the process used up said primal essence entirely, leaving nobody to follow in their footsteps. If the draeden have long-term plans, it likely lies around nurturing/preserving what remains of said primal essence, so that it can eventually regenerate itself someday. They're also very much aware that other races managing a similar ascension might leave another wasteland behind, and so subtly meddle to avoid such.)
