Usually, in AD&D 2e it would be monster HD and best THAC0/ST choice between class and monster progression. Prior to 3E monsters don't use character rules and therefore there are no hard rules for improving monsters by adding classes or templates (which of course are a 3E concept). So whenever you create a non-regular monster or NPC you basically assign whichever stats you seem appropriate and be done with it.
