IMC I'd use Will saves modified by the creature's horror rating. The result of a failed save could be temporary INT loss. If the INT is temporarily reduced to 0, the character won't die, but probably develop a permanent derangement. This doesnt work too well with your average heroic campaign however. Ofcourse, some campaigns may have acess to easy magical cures for such illnesses.
