Further infusing canon/fanon in the Central Karameikos map
This is a great source btw

Orc Totems
Orcs collect rocks, skulls and timber sections which seem to have faces. They did these together through a central pole and place the segments through a hole in the centre so they keep standing, adorning them with blood, fur, feathers tar. Sometimes skulls of heroes (to them at least) are placed on top to honor them, and/or fallen foes (to them at least) lower on the totem, clutched into makeshift claws, or pierced on added weapons. These are Orc Totems found in the Cruth Lowlands useed by the various ork tribes, they are mostly placed on minor trails along the path to the viosion quest site. Within the accompanying barrels, caches of food and usable items./weapons my be found 0- 35% chance they are used by someone, 36-80% chance they are rotten and useless to non humanoids (a very ingenious person still might find some use thinking out of the box), 81-99% they can be used by any)

Orcish vision quest site
Similarly they created the Vision Quest site. A larg treelog, some cut flagstones and some Elephant/Mastadon teeth (these beasts or even are still found in many locations on the Known World and the teeth are traded and preferred target of theft by Humanoids) or logs cut in shape to resemble such teeth. With added pieces of rusty metal shaped to resemble a vile face, hammered together with nails and Pieces of metal, adorned with skulls, feathers, paints and such. A fire is placed in the charred tree (no longer vulnerable to fire) dancing rituals are performed to the point of exhaustion and then the fire is removed, incenses and food Attuned to the specific Immortal) are placed and the orc requiring a vision sits in the warm location, sacrifices an animal, child, or easy victim to call for attention from his/her Immortal. if done right a vision for that character will be granted. mostly these will be futuristic scenes of strength, prowess and heroism, rarely they will come through though, knowing orc's chaotic tendencies and behavior.

From Lords of the Cruth Lowlands:Orcish vision quest site
This site is a place of religious significance for the nearby Orcish tribes -- encounters with Orc shamans and warriors are more frequent here. The vision quest site might be used by people of other races, provided they know the rituals and have the stomach for them. Performing the exhausting ritual dances and sacrifices required by the shamanic Orc religion leads to prophetic visions. It is also the place where Orc Shamans and Wicca come to perform the ceremonies that enable them to increase their magical prowess.

When destroyed these items will be repaired within a few days.
