Hold Breath. The monster can hold its breath for 1 hour.
Many Heads. The monster has five boar heads. While it has more than one head, the monster has advantage on saving throws against being blinded, charmed, deafened, frightened, stunned, and unconsciousness.
Whenever the monster takes 20 or more damage in a single turn, one head dies. If all fives head die the monster dies.
The process of grafting on boar heads requires the magical suppression of the usual hydra head regeneration ability.
Reactive Heads. For each head that the monster has in excess of one, it has an extra reaction that can only be used for opportunity attacks.
Wakeful. When the monster sleeps at least one of its boar heads is awake.
Relentless Madness (Recharges after a short rest). If the monster would be reduced to zero hit points, it is reduced to 1 hit point instead.
